This position is responsible to diagnose and treat human responses to actual or potential health problems through services which include case finding, health teaching, health counseling and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician, or other licensed health care provider legally authorized to do so. This position may provide direction and guidance to LPN’s, Medical Assistants, office staff and others as appropriate. This position may provide training and orientation to new staff.

Supports and assists Director of Nursing in administration of all immunizations, including but not limited to, VFC (Vaccines For Children) program. Monitors and reports to New York State, Director of Nursing and to other staff as needed and required. Responsible for inventory at all sites in cooperation with clinical site coordinators.

Provides initial screening and follow up visits to allow for the proper care of prenatal patients including NSTs, patient education and case management.

Acts as staff RN, providing leadership, advice and coverage to/for LPN staff.

Coordinates care of patients with chronic conditions/needs, including, but not limited to, Hepatitis C, MAT, Diabetes and anticoagulant management

Performs triage duties.

Coordination of prenatal referrals to specialists or Maternal-Fetal Medicine

Demonstrates professional responsibility and accountability in implementing the nursing process.

Performs procedures in accordance with established STCHCN policy and procedures.

Utilizes equipment according to STCHCN polices/manufacturers standards.

Manages and communicates patient information appropriately.

Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient.

Respects cultural customs and beliefs.

Collaborates with other members of the health care team in order to optimize patient outcomes.

Coordinates the use of human and material resources for the provision of care to assigned patients.

Identifies and provides the patient and family with appropriate information regarding assessed educational needs. Assumes responsibility for a role in the education of students, staff and community.

Demonstrates professional growth as evidenced by a minimum of ten (10) hours of related educational in-services annually with five (5) hours pertaining to the primary unit worked.

Assists in data collection for Quality Assurance Plan.

Complete pre-visit planning/chart preparation as needed.

EDUCATION/EXPERIENCE
-
Graduate of an accredited school of Professional Nursing
-
Current New York State and/or PA, RN registration or eligible for licensure
-
Successful completion of BLS and infection control training required

Previous patient care experience in the identified area preferred

Obstetrics, primary care, and/or MAT/Hep C experience preferred
