The Sangamon County Department of Public Health is seeking applicants to perform duties primarily as an R.N./Case Manager in WIC and Family Case Management/Better Birth Outcomes Programs. These duties include:
· Attend seminars, workshops, and educational trainings related to WIC and FCM for professional growth and grant requirements
· Carry out case management of social issues amongst clients and applicants focused on providing the best outcomes and possible care for case and their family. Requires home visits.
· Educate clients on importance of breastfeeding and provide referrals to breastfeeding peer counselors.
· Evaluate applicants dietary and feeding practices to identify potential risk factors, needs and concerns.
· Maintain and follow policies and procedures related to confidentiality and HIPAA regulations.
· Promote positive health outcomes for participants following internal and State of Illinois policies and procedures.
· Schedule appointments for clients and maintain client records
The work assigned to this position may not include all possible tasks in this description and does not limit the assignment of any additional tasks for this position.
Required Qualifications:
· A valid Class D Illinois Driver’s License
· Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment.
· Ability to use personal automobile with proof of current insurance
· State of Illinois registered nursing license in good standing
