Although this position is Monday through Friday from 8:30 - 5pm, our RN role is autonomous with built in flexibility based on your patients’ needs. Duties:
- Provide skilled nursing care to patients in their homes
- Perform comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and update care plans as needed
- Educate patients and their families on managing their conditions and promoting wellness
- Coordinate with other healthcare professionals to ensure continuity of care
- Assist with discharge planning for patients transitioning from hospital to home
- Provide support and guidance to patients and their families in making healthcare decisions
Skills:
- Valid RN license in the state of Colorado
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Strong clinical skills and knowledge of medical procedures and treatments
- Excellent communication and interpersonal skills
- Ability to work independently and make sound judgments
- Proficiency in electronic medical record systems, such as HomeCare HomeBase, is a plus
