SCHEDULE:

40 hours per week, may include evening and weekend hours*** Tuesday - Friday 2pm-10pm, Saturday 9am-5pm
RESPONSIBILITIES:

Follow all policies and procedures as outlined in the Pathway to Possible Personnel Policies and Procedures and the Pathway to Possible Policies to Promote Human Dignity.

Participate in agency and Department of Developmental Services training programs and attend staff meetings
-
Participate in the development and implementation responsibilities of ISP programs designed to develop personal competencies based on each resident's long-range plan.

Complete all case management documentation responsibilities and maintain the confidentiality of all information relating to the individuals supported by Pathway to Possible.

Participate in the maintenance of house, grounds, furnishings and equipment.

Be available and responsive to all P2P supervision.

Take direction from the Program Manager and participate in program and personnel evaluations.

Attend and participate in client's medical appointments and obtain necessary documentation needed for follow-up.

Must engage in personal care and hygiene maintenance of clients as necessary
Note: Staff may be re-assigned work locations based on the needs of the people we support.
Requirements:
QUALIFICATIONS/REQUIREMENTS:

High School Diploma
-
Bachelor’s Degree preferred
-
Human Service experience required
-
Valid Massachusetts Driver’s License required

Driver's record being returned as valid and acceptable
-
First Aid and CPR training required
-
Current MAP certification or the ability to acquire MAP certification (Medication Administration Program) within 90 days of your start date and maintain MAP certification for the duration of employment

Legally authorized to work in the United States

Clearance of CORI background checks as required by state and federal law
-
Clearance of DPPC registry as required by state law
-
Cleared search of fingerprinting through DDS criminal background check
