Full-Time (40 hours) AM Shift APPROXIMATE PAY RANGE: $38.98 - $47.94 Provides for Clinical case consultation for Clinical Social Worker in accordance with Board of Behavioral Science (BBSE) Guidelines. ESSENTIAL DUTIES -Effectively facilities and documents patients that are admitted/returning to Nursing Home Facilities. -Keeps abreast of current trends and developments as related to Case Management/Social Services, discharge planning, and home health. -Performs psychosocial assessments and discharge planning for high risk patients. -Participates in patient care planning responsive to age specific needs. -Refers patients to appropriate community resources. -Provides crisis intervention and emotional support to patients and families on bio-psychosocial issues. -Coordinates patient/family physician conferences. -Provides consultation to physicians and hospital staff re; psychosocial issues, bioethical issue and discharge problems. -Develops and maintains working relationships with community agencies. -Participates in planning the social work component required in selected hospital programs. -Provides in-service education for staff. -Documents pertinent contacts regarding discharge plans in the Social Services notes of the patient's medical record. -Performs basic administrative tasks related to the job; updating community resource lists, statistics, reports, and other routines as required. -Performs legal reports for suspected child abuse and elder abuse and domestic violence referrals. -Participates in professional organizations as appropriate. -Performs other job-related duties as assigned or required.
Required Qualification: 1. Current California LCSW license required. 2. Masters's degree in Social Work is required. 3. Current Basic Life Support (BLS) required. 4. Sexual Harassment/ Bullying Prevention Certification required. (1 month from hire) 5. Minimum five years MSW experience preferred. 6. Minimum three years of hospital administrative experience preferred. 7. Must possess excellent interpersonal skills, assertive, highly motivated, and exercise independent thinking, judgment, and decision-making. Ability to withstand long hours and function under stress.
