Job Description - RN Per Visit
LHH RN performs the following duties for LHH CLASS clients as assigned by the
Program Director:
- Performs pre-screening visits; assesses clients for level of care and needs. Completes forms required by waiver program(s) and forwards it to the State for processing as assigned and required.
- Performs admissions to the designated waiver program, obtaining appropriate consents and informing and educating patients regarding their rights, responsibilities, services to be delivered, and other required information.
- At admission, annually, and as required by program guidelines, or as indicated by the client’s individual needs/condition, changes in condition or request, completes all THHS forms or other required program forms in a timely fashion
- Documents client diagnoses and conditions, medications, service needs, needs for supplies, adaptive aids, home modifications, therapies, etc. and forwards the documentation to appropriate personnel for processing. Follows up to assure adequate and timely delivery of required and requested items and services, and compliance with time frames. Reports any problems or variances to the Program Director and Administrator, and to the THHS Case Manager as required by program guidelines. Assures appropriate documentation of follow up information and delivery of services as necessary.
- Assures the initiation of services or service plan changes to clients. Follows up on all requests for service to assure initiation.
- Keeps clients informed of service plan changes, and maintains liaison between the client, agency personnel, and the THHS case manager as necessary.
- Develops a schedule for supervision of client’s care plan based on the client’s individual needs at least as frequently as the appropriate waiver program guidelines require, or more frequently based on Board of Nursing standards and the RN’s nursing judgment.
- RN supervisors delegate, supervise, and monitor personal care attendants in the delivery of personal care tasks authorized by THHS under the waiver programs.
- If nursing tasks must be performed, the Director of Nursing will either schedule nursing hours or nursing delegation and the supervision of delegated nursing tasks.
- Where nursing tasks cannot be delegated, for whatever reason, or if the delegated personnel are unavailable, the Director of Nursing will assure the appropriate level of nursing care, meeting the client's identified needs for nursing care by direct deliver from licensed nurses.
- The LHH CLASS RN may, in the exercise of the RN’s professional judgment, train unpaid family members, neighbors, or other informal support in the provision of nursing tasks and personal assistance service according to waiver program and Board of Nursing guidelines. In certain instances, the licensed nurse from the HCSS agency may choose to work closely with the client's family or other informal support in the provision of certain nursing and personal care functions.
- Works closely with the Program Director in the development of appropriate service schedules and to assure compliance with all waiver program guidelines and to provide nursing judgment in determining the appropriate level of service and supervision to the client.
- Responsible for timely, accurate and compliant completion of waiver program documentation and CHHS required documentation as well as the observations, reporting and documentation of a client’s status and care.
- Responsible for personal assistance services coverage of waiver programs or
- nursing delegation clients when staff personnel is unavailable.
- Position is responsible for visits to assess and document whether the service plan
- is adequate; client continues to need the services; client needs a service plan change; attendant continues to be competent in provide the authorize tasks; and attendant is delivering the authorized tasks.
- Position is responsible to monitor and direct the operation and effectiveness of infection control according to OSHA standards.
- Shares call and makes skilled visits as necessary to Agency clients as necessary to assure CHHS clients’ health and safety.
- Position can be called upon to perform other duties as assigned by the LHH PAS Program Director, or by the Director of Nursing in conjunction with the LHH & PAS Program Director.
- LHH Class RN will consult with and take direction from the Director of Nursing for clinical skilled nursing guidance. Daily tasks and assignments, client assignments, scheduling, paperwork, etc. will be assigned and supervised by the LHH & PAS Program Director.
- Cooperates with staff members in carrying out the overall objectives of the LHH Programs, and CHHS.
Job Skills and Abilities:
Must be trained in the Intellectual Disability/Related Conditions (IDRC) Assessment and other forms required by the waiver programs. The IDRC training certification will be effective for a one-year period and must be renewed timely to maintain eligibility for the position. Must have the ability to read and interpret program regulations, follow instructions, and able to speak and understand the English language. Must have good assessment skills, ability to communicate well with people.
Qualifications:
A graduate of an approved school of professional nursing who is licensed in the state where practicing. Must complete the required training prior to completing any IDRC’s and must obtain annual certification (either through THHS training or staff trained by THHS).
