Holmes Lake Rehabilitation and Care Center is looking for a full-time MDS Coordinator for our Long Term Care Facility.
QUALIFICATIONS:
- Possess a valid State of Nebraska RN License or temporary license from the Nebraska Board of Health
- Good managerial and clinical skills
- Experience and/or educational background in MDS, Comprehensive Care Planning, and Medicare.
- Knowledge of Federal and State Regulation as well as Medicare and Medicaid Regulations.
- Ability to coordinate and organize nursing care related to the resident assessment.
ESSENTIAL DUTIES:
- Coordinates, gathers data and completes Resident Assessment Instrument in an accurate and timely manner following State and Federal regulations.
- Implements individual plans of care with interdisciplinary approach in accordance with federal, state and locak regulations and the established guidelines of Holmes Lake Rehabilitation & Care Center
- Transmission of MDS data to state agency adhering to federal and state guidelines
- Evaluates and re-evaluates as necessary resident needs and level of care by the standards and requirements of licnesure and regulations and MDS requirements. Completes coding.
- Coordinates efforts from all resources available, i.e. clients, staff, health care professionals and the care plan team to achieve optimal plan for each inidivudal plan of care. Monitors documentation to ensure documentation is present to support coding.
- Participates in the processing of Medicaid/Medicare forms and asists in the processing of additional insurance including VA required forms.
- Attends and participates in individual resident care planning meetings and other committee meetings deemed appropriate by the Director of Nursing.
- Assures that all assessments are completed in a timely manner by all departments and review for completeness and acuracy of information.
- Paricipates and assists in the overseeing of the care planning, modifying of care planning process and discharge planning.
- Monitor the quality and appropriateness of nursing care.
- Participates in the development, periodic review and evalutaiton of nursing policies and procedures.
- Participates in surveys by authorized government agencies and consultants.
- Maintains privacy and confidentiality of records, conditions and other informaiton relating to residents, other employees and the facility.
- Attends and participates in staff on-going training and educaitonal classes.
- Coordinates with therapies - Occupational, Physical and Speech - regarding resident rehabilitation.
- Administers TB testing, Hepatitis vaccines and helps with flu shot clinics when needed.
- Participates in QAPI
- Works as part of the interdisciplinary team to gather information, communicate changes, and maintain and update records.
- Must be flexible in scheduling
- Work as Charge Nurse on floow when needed.
- Other duties as assisgned.
