JOB SUMMARY:
Under the supervision of Director of the Center for Dementia Care and Care Management, the Geriatric Care Manager plays an integral part in the coordination of care assessing the older adult patient’s condition and providing them the necessary resources, options, and coordination for a healthy outcome. The Care Manager provides responsive, comprehensive assessments and direct services to the older adult population served by JFCS; primarily, though not exclusively, in the Marin region. The Care Manager is flexible, customer-service oriented and highly skilled in planning with older adults and their families for the senior’s future.
The Care Manager understands and assists in meeting budget and services goals, understands the mission and objectives of Jewish Family and Children’s Services, and works within the limitations of the Agency’s resources and performs all duties in compliance with agency standards.
COMPENSATION AND BENEFITS:

Salary Range: $75,000-$85,000 DOE

JOB DUTIES AND RESPONSIBILITIES:

Responds within 24 hours to referrals by calling the referrals source and then setting up a home visit promptly at the client’s convenience

Implements a thorough psychosocial assessment in the senior’s home, including completing a goal- oriented care plan that is given to the client and his/her family

Provides counseling, financial aid and/or case management services, based on the assessment and the Seniors at Home protocols for service

Conducts care consultations for clients and their families and converts consultations into ongoing care management, homecare, or fiduciary clients as appropriate

Updates care plans on a regular basis to meet the changing situation of the client. Adheres to agency and departmental policies regarding charting requirements and timelines, and other required documentation necessary to maintain patient files

Builds professional relationships and works closely with family members and other community representatives to ensure that the client’s needs are met

Facilitates meetings between the Associate Manager of Business Development and referral sources and community partners with whom the care manager works

Maintains at least an 80% productivity rate and meets fee goals as established in the JFCS Operating Budget and Work Plan. Sets and collects fees from clients, based on the JFCS Fee Schedule, as indicated
On Call Care Manager for Conservatorship:

In rotation with other care managers, serves as on-call care manager one week per month

If a healthcare decision needs to be made by JFCS as Conservator of Person or Durable Power of Attorney for Health Care, review client file & case notes in the Conservatorship database (EMS) or in the Care Management database (Evolv) and additional information in the PAM server – including but not limited to POLST form, code status discussions, DPOAHC documents that will inform the decision-making

Consult with Director of Conservatorship or other Seniors at Home Program Director on-call regarding the medical emergency to reach an appropriate and thoughtful decision on behalf of the client
JOB QUALIFICATIONS:

5 years of direct care management experience or MSW required

Two years’ experience providing case management to seniors required

Experience in electronic health record system is highly preferred

High level of interpersonal skills and the ability to form relationships with seniors and their families

Demonstrated basic proficiency in Microsoft Office Applications and is experienced in using the computer as a tracking, and planning tool

Ability to work independently and as part of a team

The position requires travel to client homes in the assigned region. Current California driver’s license, a reliable vehicle and automobile liability insurance is required
