Project VIDA was founded in 1992 to address the urgent needs of racial and ethnic minorities. Initially, the organization provided HIV education through street outreach services to area youth. Today, we have expanded our community scope by initiating a variety of programs: direct services for people infected or affected by HIV/AIDS and various prevention and health education programs. Job Summary:
As a certified DRS Case Manager & Behavioral Health Therapist, you’ll provide intensive client-centered home base case management services, which include coordination of in-home care and daily living assistance for HIV/AIDS disabled clients. You will work with the client’s medical provider and in-home service provider to ensure the client’s ability to maintain care in the community where they reside. The DRS case manager will provide treatment adherence counseling and assist the client in connecting to other core services. You will also provide individual and group behavioral health counseling/therapy.
As the DRS Case Manager & Behavioral Health Therapist, you can also contribute to the success of Project VIDA by lending a hand when needed, showing flexibility, paying attention to details, adapting to changes when needed, and thinking of your role as a part of the larger Project VIDA team rather than exclusive.
Job Duties:
- ·Prepare and maintain records and case files, including documentation such as clients' personal and eligibility information, services provided, narratives of client contacts, or relevant correspondence.
- Develop rehabilitation plans that fit clients' aptitudes, education levels, physical abilities, and career goals.
- Monitor and record clients' progress to ensure that goals and objectives are met.
- Confer with clients to discuss their options and goals so that rehabilitation programs and plans for accessing needed services can be developed.
- Conduct comprehensive intake evaluations and collect client bio-psychosocial history.
- You will also provide individual and group behavioral health counseling/therapy.
- Provide client assessment screening for substance use/abuse/dependence, behavioral risk, mental and physical health.
- Conduct home visits, accompany to medical appointments, and maintain monthly contact with clients.
- Confer with physicians, psychologists, occupational therapists, and other professionals to develop and implement client rehabilitation programs.
- Arrange for physical, mental, and other evaluations to obtain information for assessing clients' needs and developing rehabilitation plans.
- Analyze information from interviews, medical records, consultation with other professionals, and diagnostic evaluations to assess clients' abilities, needs, and eligibility for services.
- Develop and maintain relationships with community referral sources, such as clinics, hospitals, housing agencies Etc.
- Address barriers for the client, such as transportation, food insecurity, public benefits, and other determinants of health.
- Develop diagnostic procedures to determine clients' needs.
- Collaborate with clients' families to implement rehabilitation plans, such as behavioral, residential, social, or employment goals.
- Complete monthly billing and tracking of expenses and fees for service receivables.
- Collaborate with community agencies to establish facilities and programs for persons with disabilities.
PRIORITY QUALIFICATIONS
- Previous Medical Case Management experience is strongly preferred
- Certifications/licensure: CADC, LPC, LCPC, LSW, LCSW, or MISA desired
- Compassionate and able to relate to different clients with various needs
- BA/BS in Mental Health, Psychology, Public Health, Public Policy, Social Services, or a related field and/or two or more years of related experience
- Cultural/lifestyle sensitivity to African American, Latino, and LGBT populations required
- Knowledge of HIV care and supportive services
- Certified HIV/AIDS Tester within the first six months of employment
- Willing to work some evenings and weekends.
- Excellent communication and conflict-resolution skills
- Proficient at using Microsoft office and navigating the internet
- Reliable transportation and a valid driver’s license
Job Type: Full-time
Pay: $45,000.00 - $55,000.00 per year
