The Care Manager is responsible for coordinating and providing comprehensive case management services to clients experiencing behavioral health challenges and substance use disorders. This role involves assessing client needs during client intake, assisting with individualized treatment plans, connecting clients with appropriate resources, discharge planning, and monitoring progress to ensure effective outcomes. The Care Manager is an essential member of the integrative clinical treatment team and will work collaboratively with clients, families, facility providers, and other stakeholders to support client goals and improve their overall well-being.

Key Responsibilities:Client Assessment and Planning
- Conduct comprehensive assessments to determine clients’ behavioral health and substance use needs, strengths, and goals.
- Assist Primary Therapists in developing individualized treatment plans in collaboration with clients, incorporating their input and preferences.
- Identify appropriate resources and services to address client needs, including mental health treatment, substance use treatment, healthcare, housing, employment, and social support.
Coordination and Advocacy
- Coordinate services and resources for clients, ensuring timely and effective delivery.
- Advocate for clients’ rights and needs within various systems, including healthcare, social services, and legal environments.
- Facilitate communication and collaboration among service providers, clients, and families.
- Work collaboratively with the client and their loved ones on discharge planning.
Monitoring and Evaluation
- Monitor clients’ progress towards their treatment goals, adjusting service plans as needed.
- Conduct regular follow-ups with clients to assess satisfaction and address any emerging issues.
- Maintain accurate and up-to-date case records, documenting all interactions and interventions.
Support and Counseling
- Provide supportive counseling to clients, helping them navigate challenges related to behavioral health and substance use.
- Offer crisis intervention and support as needed, connecting clients with emergency services when necessary.
- Educate clients and families about available resources and how to access them.
Skills and Competencies
- Strong assessment and planning skills, with the ability to develop and implement effective treatment plans.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and collaborate with diverse stakeholders.
- Ability to advocate effectively for clients within various systems.
- Strong organizational and time management skills, with the ability to handle multiple cases simultaneously.
- Knowledge of behavioral health and substance use disorders, and familiarity with treatment modalities and community resources.
- Proficiency in using electronic health records and maintaining accurate records.
Required Qualifications:
- Bachelor's degree in healthcare administration, social work, psychology, or related field
- Strong communication skills, attention to detail, and ability to multitask in a fast-paced environment
- Proficiency in using electronic medical record (EMR) systems and GOOGLE documents.
- Empathy, compassion, and a client-centered approach to care
- Ability to work collaboratively with multidisciplinary teams, clients, and external stakeholders
- Flexibility to adapt to changing priorities, client needs, and clinic workflows.
1. Preferred Qualifications:
- Previous experience in a similar role is preferred
- CAT/CAS is a bonus for development and potentially leading care management and resource client groups
Compensation: $55,000 - $62,000 per year, Exempt
