ESSENTIAL DUTIES AND RESPONSIBILITIES
The LPN is responsible to the Nursing Manager, and for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized techniques, procedures, and established standards based on the scope to practical nursing, under the direction of a Registered Nurse.
- Participates as a member of the Care Team for specific patients following the concept of Patient Centered Medical Home (PCMH).
- Participates in the review of the patient’s medical records prior to a scheduled appointment, to identify if there are any actions needed to improve the patient’s health as a whole during that office visit. (Example: Due for a standardized screening test or procedure, due for an immunization, due for a follow-up lab, etc.)
- Participates in Continuous Quality Improvement activities.
- Prepares patient for and assists with examinations.
- Maintains record of vital statistics and other pertinent data of patient.
- Cleans and sterilizes instruments and equipment, and maintains stock of supplies and medications.
- Conducts specified laboratory tests.
- Performs administrative duties when necessary, multi-tasking as needed.
- Schedules patients for needed follow up, such as appointments with specialists, tests, special procedures, lab work, x-rays, social worker, etc.

EDUCATION AND/OR EXPERIENCE Graduate of an accredited LPN training program.
Drug treatment program experience preferred.
LANGUAGE SKILLS English required. Ability to speak foreign languages a plus.
MATHEMATICAL SKILLS Must be able to calculate medication dosages based on weight, chart growth on growth charts, other math skills as required for nursing duties.
REASONING ABILITY Skill in assessing health needs and diseases as state requires.
CERTIFICATES, LICENSES, REGISTRATIONS Licensed as LPN in Illinois.
