We’re currently looking for an exceptional Certified Nurse Assistant Care Manager, just like YOU to help us change the way healthcare is being delivered across the world.
As a Care Manager, you'll work closely with patients to understand their individual needs, identify potential barriers to care, and develop personalized care plans. You'll also help patients navigate the healthcare system by scheduling appointments, coordinating referrals to specialists, and assisting with insurance and billing issues.
The ideal candidate for this position will have a deep commitment to patient care, exceptional communication skills, and a passion for making a difference in the lives of others. You'll need to be a detail-oriented, proactive problem solver who can manage multiple tasks and priorities in a fast-paced environment.
Key responsibilities of the Care Manager role include:
- Building relationships with patients and their families to better understand their healthcare needs and provide compassionate support.
- Providing self-management education to ensure that patients have the tools they need to manage chronic conditions at home.
- Working collaboratively with patients and PCP to create individualized care plans that include SMART goals.
- Managed assigned patient case load to ensure that performance goals are met each month.
- Documenting each patient encounter, tracking progress in the patients’ EMR, and utilizing other software systems
- Connecting patients to appropriate resources within and outside of EPIC Health
- Proactively addressing, educating, and closing gaps in care during patient encounters
- Reviewing current medications with patients and completing medical reconciliation in order to improve compliance.
- Maintaining patient confidentiality and compliance with HIPPA
- Participating in regular process improvement projects to help improve patient care and office operations.
- Training other staff members as needed.
Qualifications for this role include:
- Must have a valid Certified Nurse Assistant (CNA) certification.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with patients and their families
- Strong organizational and time-management skills, with the ability to manage multiple priorities and tasks effectively
- Familiarity with electronic medical records and other healthcare technology tools
- Knowledge of healthcare policies and regulations, including Medicare and Medicaid
