Position Description: The Registered Nurse/Field Supervisor is responsible for developing a care plan, directing a training plan for caregivers, and providing supervision of care.
Qualifications:
- Must be a graduate of an accredited college or university.
- Must be licensed in the State as a Registered Nurse.
- Must have a minimum of one year of experience in a home health agency or in a hospital setting.
- Must have knowledge of Medicare and Medicaid guidelines.
- Must have organizational and time management skills, be knowledgeable in the principles and techniques of professional nursing.
- Must have superior communication skills.
- Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.
Duties include, but are not limited to:
- Conduct in-home assessment 48 hours or sooner after initial inquiry call.
- Develop, implement, revise, and manage the client's care plan in collaboration with the client and the responsible party, if applicable.
- Perform client and caregiver check-in calls 24 hours after the initial service to confirm that the Care Plan makes sense and that the fit between client and caregiver will work.
- Perform in-home follow-up visits 14 days, and 30 days after initial service.
- Make a supervisory home visit to each client's home at least every 60 days starting from the date of initial service or as the level of care requires to ensure that the client's needs are met.
- At supervisory home visits, assess the client's general condition, vital signs, review of the progress being made, and the problems encountered by the client and the client's satisfaction with the services being delivered. Such supervision shall also include observations about the appropriateness of the level of services being offered. Routine supervisory visits shall be made at the client's home and shall be documented in the client's file or service plan.
- Approve caregivers’ initial and yearly training plan. Conduct and proctor Basic first aid and CPR training.
- Supervise Personal Care Assistants (PCAs) and other Companions or Sitters.
- Perform annual evaluation of PCAs, Companions, and Sitters, either through direct observation or demonstration.
- Report problems and progress of client to supervisory personnel.
- Coordinate total patient care by monitoring the client’s conditions, promote sound preventive practices, and coordinate services.
- Conduct initial and ongoing comprehensive assessment of the client’s needs.
- Participate in weekly staff meetings, discuss with the supervisor problems concerning the clients and how they may best be handled.
- Complete continuous improvement trainings.
- Cooperate with other agencies to provide continuity of care.
