As Occupational Therapist, OT, you will provide excellent care to reduce or eliminate the patients’ areas of deficit associated with their sensory processing, fine-motor abilities, and muscle strengthening.
Home Care Occupational Therapist, OT
tasks will include, but not limited to:
· Occupational Therapist, OT plans and conducts individualized occupational therapy programs in a home care environment to help patients develop, regain, or maintain their ability to perform daily activities.
· Review authorized orders for therapy services, and perform and document initial evaluation to determine patients’ fine-motor deficits, sensory processing deficiencies, and cognitive perception deficits, and identify the therapeutic needs and goals specific to the patients to maintain, develop, and/or restore fine-motor function, sensory-processing abilities, and muscle strengthening.
· Perform clinical assessments to define functional impairment and developmental and/or restorative potential. Confer with patient, family, medical treatment team to determine maintenance, developmental, and restorative therapeutic goals.
· Track and document treatments, responses, and progress. Reevaluate, redefine therapeutic goals, and develop new treatment plans as necessary.
· Recommend assistive/adaptive equipment for patient to maximize independent ability to carry out daily functionality.
Occupational Therapist, OT - SNF
tasks will include, but are not limited to:
· Occupational Therapist, OT plans and conduct individualized occupational therapy programs in Skilled Nursing Facility to help patients develop, regain, or maintain their ability to perform daily activities.
· Review authorized orders for therapy services, and perform and document initial evaluation to determine patients’ fine-motor deficits, sensory processing deficiencies, and cognitive perception deficits, and identify the therapeutic needs and goals specific to the patients to maintain, develop, and/or restore fine-motor function, sensory-processing abilities, and muscle strengthening.

Qualifications:
- Must have COVID-19 Vaccination - 2 shots
- Occupational Therapist NY license required

Pay: $42.00 - $80.00 per hour
