Description:
Reporting to the Director of Community Health, the PATHS (Positive Aging Through Home Supports) Wellness Nurse (RN or LPN) will focus on building trusting relationships with participants in PATHS, in order to provide oversight of wellness care and coaching. They are responsible for overseeing the well-being of participants and for coordinating health services with other members of the PATHS team and community providers.

Provides education on identification of early warning signs for quick interventions of chronic conditions.

Emphasizes the importance of and provide encouragement to participate in activities that promote good health and decrease symptoms.

Promotes the importance of keeping an open line of communication with medical providers.

Coaches participants on the importance of scheduling and attending preventative and follow-up appointments.

Makes in-person visits with participants shortly after medical appointments, hospitalizations or procedures to improve adherence to health recommendations.

Attends medical appointments with participants when appropriate to provide clarity and support.

Offers regular wellness screens to include: blood pressure checks, heart rate monitoring and weight monitoring. Monitor participants overall physical and mental status.

Partners with Health Educator to plan, prepare, and promote regular PATHS wellness programs.

Participates in regular PATHS team meetings ensuring optimal engagement and process improvement.

Facilitates Agency selected evidence-based programs in additional community settings with team members and volunteers.

Attends Agency In-service programs.
Current Schedule: 20 hours per week, Tuesday & Wednesday: 8:30 AM – 4 PM with 30-minute meal period. There is the potential to change days of the week based on candidate availability. Facilitation of evidence-based programs will occur as needed on additional days. Occasional evening or weekend work hours based on Agency needs may be required.
Requirements:
- RN or LPN with public health nursing experience preferred

Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)

3-5 years of experience working with older adults and in public health nursing preferred

Excellent time management and organizational skills (ability to prioritize daily work in a prescribed order that promotes efficiency, timeliness and accuracy)

Excellent customer service skills (friendly, courteous and helpful)

Able to understand and carry out directions and instructions

Able to work with minimal supervision
