Full Job Description:
The Visiting Registered Nurse plays a vital role in our home care agency by contacting our patients and visiting them to assess their health to verify and adjust the type of services they need and how we can accommodate them. The Visiting Nurse documents the patient’s service needs as well as any changes in their condition so we as their homecare provider can provide them with the most appropriate care and services possible. The Visiting Nurse serves as a liaison between the client, their family, our agency, and the insurance to adjust and verify if the patient is receiving the proper types and quantity of services.
Responsibilities:
- Make planned home visits to patients to assess, evaluate, and provide direct nursing care.
- Assess the patient/family situation for nursing and health-related problems to create medical orders.
- Develop and coordinate the plan of care specific to the needs of the patient.
- Develop and implement a comprehensive care plan for each patient and make appropriate adjustments by the patient’s physician and/or care team.
- Instruct and supervise patients, caregivers, nurses, and home health aides in aspects of patient care as appropriate.
- Report and document significant conditions/symptoms to the patient’s physicians and modify the plan of care accordingly.
- Educate the patient/family regarding the disease process, self-care techniques, and prevention strategies.
- Evaluate the effectiveness and outcomes of care.
- Document all nursing (home care) services provided to the patient as well as periodic reports to coordinate care and remain in compliance with state and federal regulations.
- Prepare and maintain clinical progress notes according to agency policies and acceptable nursing standards.
- Manage document flow and confidentiality of patient records as per Agency policy.
- Perform other duties as assigned.
Requirements:
- RN Certification in the state of NY.
- Excellent communication skills.
- Excellent time management skills.
- Knowledge of the RNPad Platform is preferred but not required.
- Ability to master basic computer skills.
- Willingness and ability to travel between boroughs.
- Disciplined and flexible.
- Multi-lingual encouraged to apply (Spanish, Chinese, Russian, Haitian Creole)
