The LPN Discharge Planner will be responsible for coordinating and planning the discharge process for patients. They will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the appropriate care and resources upon discharge. The LPN Discharge Planner will also be responsible for:
- Assessing patient needs and developing a discharge plan
- Coordinating with homecare agencies and other resources to ensure that patients have access to the necessary equipment and services
- Providing education to patients and their families on post-discharge care and resources
- Documenting patient information and discharge plans in electronic medical records
- Collaborating with healthcare professionals to ensure that patients receive appropriate follow-up care
The ideal candidate will have strong computer skills, knowledge of durable medical equipment (DME), and experience working with homecare resources. They should also have excellent communication and organizational skills, as well as the ability to work independently and as part of a team.
