Description
Support and provide service to individuals with intellectual disabilities and health care needs that enhance their quality of life as they live, learn and work in their communities and environments of their choice. DSPs with the guidance of manager/Supervisor/Field Trainer follows the ISP/Care Plan. Provide preventive and wellness care in settings comfortable to individuals/participants to which they serve.
DSP’s are a necessary component of a quality health, support, and long-term care workforce that ensures public health and safety. They are an essential member of a diverse interdisciplinary team and an important role as the health, support, and long-term care delivery systems shift service delivery from more restrictive settings to include more home and community-based services. They support and provide service to individuals with disabilities and health care needs that enhance their quality of life as they live, learn, work, and recreate in communities and environments of their choice.
Qualifications/Educational Requirements:
- Graduate of an accredited High School or equivalent preferred.
- Ability to read, write and follow directions.
- A minimum of one year full-time experience in direct consumer in an institutional setting (hospital or nursing facility) or one year full-time experience in direct client care in an agency setting.
- Work positively and favorably with consumers, families, and staff.
- Demonstrate compassion, responsibility, and cheerful attitude.
- Listed on the nurse aide registry with no finding against the aide relating to client abuse or neglect or misappropriation of client property.
Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.
- Follow the consumer ISP as directed by manager/supervisor/trainer and on the care plan
- Maintain Daily Progress Notes
- Provide assistance with hygiene such as bathing, oral care, dressing, errands and other personal care per care plan/ISP
- Observe and report any safety hazards found in the client’s home or any significant observations regarding the client.
- Complete annual in-services
- Report and document injuries immediately to supervisor
- Maintain Consumer/Client confidentiality/adheres to HIPPA requirements and agency policy and procedures manual.
- Possess the ability to follow written and oral instructions.
- Report abnormal findings in consumer’s conditions as observed and per care plan.
- Perform the following task
- Ambulation, positioning, and turning
- Transportation
- Personal hygiene
- Feeding
- Socialization
- ADLS

Pay: $13.75 - $15.00 per hour
