CDPAP COORDINATOR- Bi-lingual

POSITION SUMMARY:
CDPAP Coordinators are responsible for processing referrals received from marketing sources such as hospitals, nursing homes, and community physicians and online campaigns. Coordinators contact each patient to educate them on the CDPAP services Preferred provides and accommodates patients with their homecare needs for skilled and non-skilled services. The CDPAP Coordinators top priority is to keep the patient and family members informed at all times to ensure a caring and professional experience.
QUALIFICATIONS:
- Two years of experience within the Home Care services industry; Intake Coordinator experience preferred
- Proficient in Microsoft Office Suite and HHA Exchange
- Understands the regulations governing the homecare field, related to Medicare, Medicaid and other insurance.
- Understanding of communicating effectively with employee, clients and their families, medical and community affiliates in order to develop positive relationships.
RESPONSIBILITIES
- As a CDPAP Coordinator, you will play an integral part in the agency
- This CDPAP Coordinator position calls for assisting with day to day operations of the coordination department
- Documents all notes and updates
- Keeping all electronic records up-to-date, and in strict compliance with agency policy and regulations

Pay: $21.00 - $23.00 per hour
