Job Summary
A licensed practical nurse or a registered nurse who has knowledge of medical chart data collection, and completion of the Minimum Data Set. Demonstrates knowledge and understanding of the CMI and regulatory requirements while maintaining confidentiality of all information. Coordinates and communicates with nursing and therapy.
Major Responsibilities, Duties, Functions, and Tasks of the Position
- Review medical records related to residents health status and examine residents to collect information related to minimum data set assessment
- Completion of accurate MDS in accordance with regulatory guidelines
- Coordinate Assessment Reference Dates and Discharge Dates with the Director of Nursing
- Team with Rehab Services to provide services to residents as needed, within set dates to maximize resident services and CMI
- Manage weekly CMI meetings w/ Interdisciplinary Team.
- Attend morning meeting and Medicare Meeting to monitor residents' condition
- Submit long term care insurance information
- Conduct CMI report review and validate for accuracy
- Maintain appropriate documentation to comply with UMR audits.
- Serve as the contact point for UMR
- Maintain ongoing CMI projection to assist Admissions Coordinator in monitoring a benchmark CMI.
- Maintain ongoing CMI projection to assist CFO in reimbursement projection for future quarters.
- Maintain required knowledge about regulatory changes to the MDS.
- Participate in daily meetings to identify potential changes in resident acuity which would initiate modified MDS reporting.
- Immediately report resident related care concerns about treatment or abuse to the Department Supervisor as well as the Executive Director of the facility
Holds current LPN or RN license
