Hospice of the Piedmont is dedicated to providing compassionate and exceptional end-of-life care. We are currently seeking a full-time Hospice Nurse Liaison to join our team. The Hospice Nurse Liaison plays a crucial role in coordinating services for individuals inquiring about or referred to hospice services.
As a Hospice Nurse Liaison, you will be responsible for providing the initial hospice presentation to patients and their support circles, advocating for hospice admission of medically eligible patients, and promoting the gift of hospice within the extended care community through customer service and education.
Responsibilities/General Duties:
- Develop a hospice presence in hospitals, attend rounds and care conferences, and actively participate in in-service meetings.
- Coordinate timely discharge plans for newly referred patients, assess appropriateness for hospice admission, and inform patients and families about Hospice of the Piedmont services.
- Educate and serve as a consultant to hospital medical professionals on hospice services, pain management, and reimbursement.
- Provide input into marketing plans and contribute to census growth.
- Act as a liaison and advocate for current hospice patients admitted to the hospital.
- Maintain and submit records of time spent, referral and admission statistics, and related activities.
- Participate in research, quality improvement, and other studies involving end-of-life care.
- Graduate of an accredited nursing school; licensed as a Registered Nurse in the Commonwealth of Virginia.
- Two or more years of relevant nursing experience in terminal care, health team service coordination, documentation, and case management.
- Previous experience in hospice, home health, healthcare sales, or marketing.
- Strong knowledge of hospice regulations and reimbursement issues.
- Ability to manage all aspects of terminal care, health team service coordination, and documentation.
- Emotionally mature and caring individual willing to provide support to terminally ill patients and their families.
- Basic to intermediate proficiency in word processing software and basic computer skills.
