Job Description: As an Experienced PRN Home Infusion Nurse, you will play a crucial role in administering infusion therapy to patients in their homes. Your expertise in intravenous medication administration and patient education will contribute to the safe and effective management of complex therapies. You will collaborate closely with physicians, pharmacists, and other healthcare professionals to ensure seamless care delivery and optimal patient outcomes.
Responsibilities
- Perform comprehensive assessments of patients' infusion therapy needs and develop individualized care plans.
- Administer intravenous medications, fluids, and nutrition therapies according to prescribed protocols.
- Monitor patients for adverse reactions and provide timely intervention as needed.
- Educate patients and caregivers on infusion therapy procedures, medication administration, and potential side effects.
- Ensure accurate documentation of patient progress, medication administration, and clinical outcomes.
- Collaborate with multidisciplinary teams to coordinate care and communicate patient updates effectively.
- Adhere to all regulatory standards, policies, and procedures related to infusion therapy and home health services.
- Delivers appropriate care to patients, at a competent level, according to the Plan of Care.
- Coordinates the planning for delivery of nursing care with Director of Nursing and administers appropriate clinical decisions in the delivery of patient care.
- Proactively updates client/patients’ POC, health changes, and other related incidents.
- Ensures safe care to patients; adheres to all policies, procedures, standard within budgetary specifications including time management, supply management, productivity, and quality of service.
Requirements
- Current unrestricted Nursing license in the State of Texas
- Certified CPR and negative TB skin test or chest x-ray
- Clean background and criminal records
- Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility, preferred.
- Licensed driver with valid automobile insurance
- Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
