provides direct nutrition counseling services to patients and their families. Registered Dietitian/Nutritionist (RDN)is responsible for helping patients and community members improve their health and well-being. The RDN will provide Medical Nutrition Therapy (MNT) counseling to individuals and groups through telemedicine and in person as appropriate. This entails creating nutrition care plans using the Nutrition Care Process to specify diet plans or goals for patients with varying medical diagnosis such as diabetes, obesity, heart disease and more. The RDN may counsel individual patients or their families in one-on-one or group settings.
Other responsibilities may include teaching nutrition and wellness classes, leading and/or overseeing cooking demonstrations, creating and maintaining nutrition education materials, co-facilitating group visits, developing community programming, promoting worksite wellness among employees, and other responsibilities as the job and need for nutrition expertise evolves in this practice setting. The RDN interacts with the medical, nursing, and social staff on developing patient care plans which are effective and culturally appropriate, including having the knowledge of nutrition and food of ethnic groups the health center services. The RDN promotes patients’ self-management and empowers patients/families to achieve maximum levels of wellness and independences.
Reporting to the Program Manager for Nutrition Services, the Registered Dietitian Nutritionist (RDN) oversees provides direct patient care to patients across the lifespan, from diverse backgrounds for a variety of outpatient nutrition conditions.
Provide medical nutrition therapy and nutrition education and counseling to CSHC patients
Counsel patients who come to CSHC via scheduled visits, warm handoffs from providers, integration into primary care clinics, and telehealth
Provide both one-on-one care and conduct group visits and classes
Participate in case management of high-risk patients
Participate on multi-disciplinary teams to optimize patient care
CSHC Staff and Patient Engagement
Collaborate with PCMH Project Manager and PCMH team.
Ensure program and team is focused on the elements of PCMH.
