MDS Coordinator - LPN (Ask about our Sign-On Bonus)
We are seeking a dedicated and experienced MDS Nurse to join our team. As an MDS Coordinator, you will be responsible for conducting comprehensive assessments of patients' health status and developing individualized care plans. The ideal candidate will have a strong background in nursing with a focus on geriatric or long-term care, along with excellent assessment, documentation, and communication skills.
Responsibilities (include but not limited to):
- Conduct comprehensive assessments of patients' physical, cognitive, and psychosocial status.
- Develop and update individualized care plans based on assessment findings and resident needs.
- Ensure compliance with Medicare and Medicaid regulations related to reimbursement for skilled nursing facilities.
- Maintain accurate and thorough documentation of patient assessments, care plans, and interventions.
- Collaborate with interdisciplinary team members, including physicians, therapists, social workers, and administrators.
- Stay updated on state and federal regulations governing long-term care facilities and ensure compliance in all aspects of MDS responsibilities.
- Participate in continuing education and professional development activities to stay current in the field.
Qualifications:
- Must have a Current LPN License.
- Previous experience in nursing, preferably in skill-nursing or long-term care settings.
- Strong assessment skills with the ability to conduct comprehensive patient assessments.
- Knowledge of Medicare and Medicaid regulations related to skilled nursing facility reimbursement.
- Proficiency in documentation and record-keeping.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
