Days/Hours: Monday Friday, generally 9:00am to 5:00pm
Position Summary:
The Fortune Society is seeking an innovative, client-centered, strategic thinker to join our Behavioral Health team. This position will be under BH's new SAMHSA Grant to Benefit Homeless Individuals (GBHI). The Housing Specialist will have overall day-to-day responsibility for connecting criminally justice-involved patients, with complex health conditions, to the full range of medical, mental, and social determinants of health services they require, including housing screenings and referrals. This is a growth position for the right candidate who demonstrates a strong work ethic and a willingness to think outside of the box for the betterment of his/her assigned caseload.
Essential Duties and Responsibilities:

Coordinate Plan of Care for clients with criminal justice histories and chronic medical and/or mental health conditions that are homeless or at risk for homelessness;

Screen all participants assigned for eligibility into Fortune housing;

Offer supportive services such as, peer services, medication assistance/linkages to healthcare housing screenings and assistance with the HRA 2010e Supportive Housing Application;
- Conduct and perform data entry for GPRA and SAMHSA Cross-Site surveys/evaluations at intake, 6-month follow-up, and discharge, maintaining appropriate follow-up rates;

Work closely with Fortune’s interdisciplinary care team including the PCP, psychiatrist, therapist, residential service providers, DHS case workers, substance abuse treatment programs, ACT teams, HCBS partners, care team, and/or family members, etc., to determine appropriate service referrals;

Under supervision of the director, ensure that all services delivered are within the scope of the Plan of Care and that accurate records of needs/progress/regress are maintained, as per NYS DOH Medicaid guidelines;

Include input from providers/client/client’s family for written individualized Plans of Care and consult with family members, parole and probation as necessary;

In conjunction with the client and Behavioral Health and Care Management team, identify potential barriers to care and strategies for overcoming those barriers, including those relating to medication compliance;

Regularly liaise with Fortune’s housing screening team to identify potential placement opportunities for eligible individuals;

Participate in conferences, workshops and other professional development activities to remain current with advances in field of expertise;

Perform additional responsibilities as needed.
Qualifications:

MSW preferred, though a minimum of a Bachelors degree is required, preferably in social work, criminal justice, community mental health, or related health services;

Minimum of 1 year of experience in healthcare, social work, or mental health field preferred; college graduates with strong internship or field experience will also be considered;

Ability to demonstrate strong written and verbal communication skills through reports, correspondence and presentations;

Strong organizational and interpersonal skills; ability to manage competing priorities within a growing unit;

Proficiency in MS Office, SharePoint, and Excel (working knowledge of MAPP, ePaces, PSYCHES and Foot Hold plus, SPARS);

Bilingual English/Spanish strongly preferred in order to best suit the clients who reside in this area;

Ability to perform additional responsibilities as needed;

Travel Requirements Will require travel throughout the five boroughs, as well as to Fortunes locations in Harlem and Long Island City. May require additional travel to clients’ locations, including those within the shelter system, when necessary, to ensure continued progress of Plan of Care. MetroCard transportation is provided to staff for all client-related activities.
