- must speak English, Cantonese and Mandarin
-Conducts case work and apartment/medication inspections for supportive frail elderly housing program in Manhattan.
- Maintains current records of all resident medications including securing signed doctor's for each medication and each change in medication as it occurs.
- Assists Enriched Housing Program Director with administrative duties as needed, and help to ensure programmatic compliance with the New York State Department of Health's Enriched Housing rules and regulations.
- Links residents with service providers in the community; home attendants/home health aides, meals-on-wheels, transportation, visiting nurse etc.
- Educates residents on service availability, providing advocacy as appropriate.
- Creates monthly activity schedules and secures speakers for informational series and entertainers for daily activities.
- Assists residents with applying for benefits and entitlements.
