· Consult with primary care providers, specialists, and other disciplines to resolve medical necessity concerns raised by the patient/family
· Educate patient/family about the patient’s medical needs and diseases
· Review and set self assessments goals with each patient
· Actively participates in the interdisciplinary health care team
· Identifies community resources which can provide assistance to the patient/family
· Develops and documents care plans in the patient chart
· Provide emotional support to patient and their family
· Work with the care team to stratify the level of disease complexity for each patient.
Medical experience required. Prefer Care Coordinator, Certified Medical Assistant or LPN.
