Presbyterian Medical Group seeks a Hospice Care nurse practitioner to join our established practice in Albuquerque, New Mexico. We’re all about well-being, starting with yours. Practice Highlights
•Services as a hospice provider, offering clinical oversight on care of hospice patients, both inpatient and outpatient.
•Assists Hospice Medical Director with documentation for certifications to obtain hospice benefits for patients.
•Collaborates with colleagues in the management of patients, both in the hospital and outpatient settings
•Offers care to patients with critical and life-limiting illness, focused on assisting them to achieve medical treatment concordant with their goals.
•Builds effective relationships with referring physicians and care teams, strives to improve collaboration centered on the care of these patients.
What We’re Seeking
•Master’s degree in nursing, New Mexico Nurse Practitioner License, ANCCP or AANP certification required
•Outstanding patient care qualities, highly motivated with an interest to grow the practice
•Patient-focused and willing to collaborate and work in a team environment
•Ability to obtain a medical license to practice in the state of New Mexico
