JOB SUMMARY: Working with a team providing wrap around services to OASAS Continuum of Care (CoC) program is responsible for, but not limited to intake, continuous assessment, and supportive case management for clients living in scatter-site apartments.
DUTIES AND RESPONSIBILITIES:
· Coordinates move-in for new clients and relocation for existing clients when necessary
· Assists clients with establishing utility services and monitoring monthly payments to ensure utilities are not disconnected for non-payment.
· Conducts intake assessments, quarterly re-assessments, monthly (at minimum) home visits, and develops and implements initial, annual, and quarterly service plans with each client on caseload of 20-25.
· Complete DLA-20, Trauma Screening, and other mandatory agency forms in a timely manner.
- Monitor clients' progress toward their goals via regularly scheduled home visits and telephone contact, and document visits via progress notes.
· Work with clients to break through barriers to service goals and to assist clients in advocating for themselves and in moving toward self-sufficiency.
· Calculates, collects, and tracks monthly rent payments from each client to ensure each client makes appropriate monthly rent payments and manages delinquencies as appropriate.
· Assist clients receiving public assistance with completing and submitting through SSI/SSDI application packet using the SOAR model.
· Resolve all apartment repair/maintenance needs via BAC maintenance staff and advocacy with clients’ superintendents, building managers, and landlords.
· Participate in internal and external case conferences.
- Utilize person center approach and motivational interviewing to aid clients to meet their prioritized needs.
· Other related duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
· Excellent organizational and record-keeping skills, as well as the ability to manage multiple cases
· Knowledge of issues related to substance use, recovery, and mental health.
· Excellent computer skills, including proficiency in Microsoft Office (Word, Teams, Excel) and the ability to learn new programs.
· Familiarity with electronic medical records (EMR).
· Excellent written and verbal communication skills.
EDUCATION AND WORK EXPERIENCES:
· Minimum HS Diploma or equivalent with 2+ years of field experience providing housing and/or case management services to homeless populations, people with substance use disorders, and/or people with psychiatric illness.
· Bachelor's degree in social work, Human Services, or related field, plus 1 year of experience providing housing and/or case management services to homeless populations, people with substance use disorders, and/or people with psychiatric illness strongly preferred.
