Your Role:
- Schedule: Monday through Friday, full-time days
- Key Responsibilities:
- Align all activities with the vision of Elara Caring’s leadership.
- Assess hospice patients and families to identify their needs.
- Manage a caseload, including planning, coordinating, and evaluating treatment plans.
- Conduct initial and follow-up patient evaluations.
- Communicate significant findings and changes to the Clinical Supervisor and other relevant personnel.
- Revise care plans as needed and provide counseling and instruction to patients and families.
- Teach hospice philosophy and pain management to staff in various care settings.
- Supervise home health aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Provide support at the time of death and perform bereavement assessments.
- Document medication regimens and maintain confidentiality in compliance with HIPAA.
Requirements:
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State Licensee as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in hospice or home health environment is preferred
- Nursing skills as defined by accepted nursing standards
- Ability to handle multiple priorities, documentation requirements, and deadlines
- Reliable transportation to perform job tasks
- Preferred to live in the service area (less drive time for you!)
