KCHC/Pillar Health is looking for experienced registered nurses to support it's intake department.
POSITION SUMMARY
The Intake Registered Nurse (IRN) will be the first point of contact between the care team and the patient and will ensure initial clinical, social, and financial needs are identified and addressed and that the patient information is handed off to the direct care team to support a high-quality, efficient, and effective visit. The Intake RN will spend a significant amount of time with the patient either over the phone or in person collecting medical history, discussing medications, evaluating social needs impacting health, initiating a plan to address barriers, and completing other evaluations as need. The IRN will evaluate the collected information to determine intensity of services required and appointment type and length. The IRN will begin the patient care plan with referrals to ancillary staff as needed. The IRN is a dedicated professional who handles basic triage and acclimates the patient to the Pillar model. A successful admission nurse will be skilled at “processing” patients and sending them off to the team for continued care. The intake area is fast paced and requires good judgment to determine the initial course of the patient’s care.
ESSENTIAL FUNCTIONS
The list of duties and responsibilities is not all inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.
Initial Patient Assessment and Triage
· Performs initial registration of the patient utilizing the registration workflow including demographics, communication preferences, veteran status, homelessness status, seasonal worker status, insurance information, and sliding fee scale discussion.
· Collect and document patient clinical data including, but not limited to presenting problem, past medical history, surgical history, dental history, behavioral health history, current medications, and allergies,
· Collect and document emergency contact information and ensure that patient demographics are correct.
· Collect, document, and evaluate social needs impacting health and documents utilizing standardized SDOH tool consistent with organizational policy.
· Evaluate collected data to determine the appropriateness of the date of the scheduled appointment and triages patient to another care setting if appropriate.
· Work with designated Emergency Departments to receive referrals for patients who need to establish or re-establish with the Center for continued care needs.
Initiation of the Plan of Care
· Initiates a care plan that addresses clinical, psychosocial, social needs as identified during the patient interview.
· Understands the internal and community resources available to meet those needs, and places referrals for services as needed prior to patient’s appointment i.e., case management for SDOH gaps.
Team Coordination and Communication
· Coordinates pre-visit activities with non-clinical staff to ensure clinical, financial, and social needs planning completed and well-documented.
· Complete warm hand off as needed to the medical, dental, or behavioral provider either during huddle or prior to services starting.
Other Responsibilities
· Document all information correctly in the EHR.
· Adhere to general organizational, department, and clinical policies.
· Adhere to patient confidentiality and HIPPA requirements.
· Attend all mandatory training sessions and staff meetings as assigned.
· Conform to infection control policies, utilize aseptic techniques, and follow standard precautions.
· Serve as a clinical resource to ambulatory directors, managers, and staff.
· Communicate effectively with colleagues across Kenosha Community Health Center.
· Demonstrate skill in navigating and communicating in a complex environment.
· Actively participates in Professional Nursing Practice Committees and organizational workgroups
POSITION REQUIREMENTS
Education and/or Experience
· Graduate of an accredited university or college of nursing. Bachelor’s degree preferred.
· Minimum of 2-5 years’ experience in a professional nurse role, with 1 year in an outpatient setting preferred.
· Experience with Electronic Health Record preferred.
License/Certification
· Licensed as a Registered Nurse in the State of Wisconsin.
· Current CPR Certification.
Physical/Environmental
· Intermittent physical activity including standing, walking, bending, reaching, and prolonged periods of sitting.
· Required to constantly operate computer, phone, and other clinical tools and/or equipment.
· Must be able to push, pull, or carry 25 lbs.
Knowledge, Skills, and Abilities
· Knowledge, skill, and application of electronic medical record required.
· Demonstrate a high degree of professionalism and service excellence.
· Ability to make knowledgeable clinical assessments and judgments.
· Excellent verbal and written skills required.
· Experience in developing care plans required.
· Ability and experience in monitoring nursing practice trends and making changes in clinical practice preferred.
· Must possess critical thinking skills, initiative, the ability to work independently and the ability to prioritize tasks.
· Ability to maintain confidentiality of medical records.
· Ability to prioritize tasks and to work in a changing/dynamic work environment.
· Knowledge of infection control.
WORK SCHEDULE
Open availability to work a minimum of 40 hours during clinic operating hours, Monday – Thursday, 7am – 7pm, Friday, 7am – 5pm and rotating Saturdays 7am – 3pm.

$5,000 Hiring Bonus*
*Full-time Registered Nurses hired between January 1, 2024 and July 31, 2024,will be eligible to receive a hiring bonus paid in 2 equal installments, paid after completion of 6 months and 12 months of service. Employee must be actively employed to receive payout. Internal candidates are not eligible.
