Job description
The Rehabilitation Registered Nurse (RN) plays a crucial role in the multidisciplinary team, providing specialized nursing care to individuals undergoing rehabilitation. Working in collaboration with physicians, therapists, and other healthcare professionals, the Rehabilitation RN contributes to the development and implementation of individualized care plans aimed at promoting recovery and maximizing functional independence. This position requires strong clinical skills, compassion, and a commitment to facilitating the holistic well-being of patients during the rehabilitation process.
Key Responsibilities:
Assessment and Planning: Conduct comprehensive nursing assessments to evaluate patients' physical and psychological status.
Collaborate with the healthcare team to develop individualized rehabilitation care plans based on patient needs and goals.
Patient Care: Administer and monitor medications, treatments, and interventions as prescribed.
Provide skilled nursing care, including wound care, catheter care, and monitoring vital signs.
Assist in therapeutic activities and exercises as directed by rehabilitation therapists.
Foster a supportive and encouraging environment to enhance the rehabilitation process.
Collaboration: Work closely with physical therapists, occupational therapists, speech-language pathologists, and other team members to coordinate patient care.
Participate in interdisciplinary meetings to discuss patient progress and adjust care plans accordingly.
Patient and Family Education: Educate patients and their families about the rehabilitation process, including expectations, exercises, and activities of daily living.
Provide guidance on post-discharge care and resources to support continued recovery at home.
Documentation: Maintain accurate and up-to-date patient records, documenting assessments, care provided, and progress notes.
Ensure compliance with regulatory standards and institutional policies.
Advocacy: Advocate for patients' needs and preferences within the interdisciplinary team.
Collaborate with case managers and social workers to address discharge planning and post-rehabilitation support.
Qualifications:
Current and valid RN license in the state of practice.
Minimum of 2 years of nursing experience, preferably in rehabilitation or a related setting.
BLS (Basic Life Support) certification.
Strong assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.
Knowledge of rehabilitation principles and practices.
Working Conditions: The Rehabilitation RN may work in hospitals, rehabilitation centers, or long-term care facilities. This position may involve standing for long periods, lifting, and assisting patients with mobility.
