Behavioral Crossroads, LLC is seeking an LPN to join our partial care and behavioral health home team! This position is in-office and can be part-time or full-time, with a minimum of 24 hours per week.
The LPN is primarily responsible for assisting the agency’s Advanced Practice Nurse in the provision of psychiatric services to agency clients. This position will also be responsible for providing wellness activities to program participants, and for overseeing the company's Infection Control program. The LPN coordinates with providers in the community who are involved with clients in the Behavioral Health Home program and assists with screening clients for eligibility. The LPN participates in treatment plan creation/updates with the Program Director and Program Supervisor’s guidance and assists with care transitioning of program participants. The LPN also completes any necessary clinical and/or administrative documentation as necessary and may provide back-up clerical and clinical support to agency administration as assigned. The LPN administers medical injections as needed, and assists with coordinating transportation for clients to appointments and services. The LPN monitors the clients’ health progress, and is responsible for meeting and exceeding contracted commitments including budgeted volume of service.
Job Responsibilities include, but are not limited to:
- Assists Nurse Practitioner during psychiatric rounds and maintains log regarding outcomes
- Maintains documentation to ensure all clients are seen for psychiatric evaluations, medication monitoring, and follow up appointments
- Phones/faxes/electronically sends prescriptions to pharmacies at the discretion and direction of the Nurse Practitioner
- Accepts/signs for medication delivered by pharmacy vendor(s) and places in locked medication cabinets. Arranges for return of outdated, unclaimed, or unused medication/needles to the appropriate vendors
- Administers prescription injections as required
- Facilitates groups and activities as assigned
- Contacts insurance companies to obtain authorizations for prescriptions
- Oversees the company's Infection Control program
To be considered for this position, you must have the following Qualifications:
- Licensed in New Jersey as an LPN (Licensed Practical Nurse)
- Prior work experience with mentally ill or cognitively challenged individuals is preferred
