Montare Behavioral Health is currently looking for a Case Manager for the Mental Health Program! This position is located in the San Fernando Valley.
The Case Manager is responsible for providing direct service to clients, conducting process and psycho-educational groups and for facilitating the smooth day-to-day operation of the program. The Case Manager collaborates as a member of the multidisciplinary team (Treatment Team) to deliver effective treatment.
Essential Duties and Core Competencies:

Support clients in accessing community resources

Make appropriate referrals for clients to other services in coordination with the Clinical Team

Conduct process and psychoeducation groups

Complete documentation in EMR

Coordinate with other treatment providers

Demonstrate appropriate boundaries with clients

Supports clients by utilizing outcome measures
-
Assist Clinical Director with Community Care Licensing requirements

Demonstrate effective crisis intervention
-
Teach independent living skills effectively
-
Utilize individualized approach to client services
-
Support clients who have discharged with follow up calls

Advocate for clients
-
Participate in staff meetings
-
Utilize supervision with the Clinical Director to enhance clinical skills
Education and Work Experience:
- Masters Degree preferred.

ACSW a plus!

One year full-time experience working directly with clients in a program serving people with mental illness.

One year of Case Management experience in a mental health or substance inpatient or residential setting.
Knowledge and Skills:
- Knowledge of and belief in the principles of client centered treatment.
- Knowledge of medications and interventions preferred.

Demonstrated ability to work independently and as a team player.
- Excellent communication skills both oral and written.

Able to maintain a professional manner with clients, staff, clients and visitors.

Able to work in a fast-paced setting, under pressure and maintain open, and proper communication.

Demonstrate professionalism and strong boundaries with the clients.

Must be fluent in speaking, reading, and writing English.
Certificates, Licenses, Registrations:
- Current AMFT, ASW, APCC or equivalent Preferred.

Valid driver's license, clean driving record and personal automobile insurance.
Pre-Employment Requirements

Hiring is contingent upon completion of a background check, TB test, and clearance on a drug screen and pre-employment physical. Clean driving record and current first aid and CPR certification or willing to obtain upon hire. Additional requirements include receiving DOJ/FBI fingerprint clearance or an exemption and a submission of a signed Criminal Record Statement (LIC 508). Clean Background is a must.
