Duties and Responsibilities:
- The Enhanced Care Management (ECM) Lead Care Manager provides and delivers services for ECM outreach, assessments, and care management plans for all patients to determine the most appropriate treatment and level of care required.
- Develops and conducts clinical documentation for ECM.
- Work with staff and patients to ensure a healthy and effective therapeutic community environment in the ECM facility.
- Provide 1:1 crisis intervention and de-escalation services with ECM Licensed Consultant.
- Coordinate care with other medical/health/social services professionals and partners as needed.
- Facilitate referrals to agencies and resources in the community for physical health, mental health, oral health, substance use disorder treatment, housing, and community-based social services.
- Provides referrals, linkages, and follow-up for services.
- Provides referrals, linkages, and follow-ups for community resources and services that can maximize independence and support recovery goals; including referrals to local food banks and/or community churches for groceries and meals; clothing assistance; transportation services; vocational services; support for employment; and education.
- Coordinates services with community-based organizations to provide health promotion as well as with the county and state.
- Assist patients in applying for public benefits and legal documentation.
- Document and manage all ECM services and protocols to electronic health records within 72 hours.
- Attend staff clinical training and daily assignments as assigned.
- Report and work closely with the ECM Licensed Consultant to ensure care coordination and collaboration between physical health, mental health, oral health, substance use disorder treatment, housing, and community-based social services.
Minimum Requirements:
- Post-Secondary education degree or certificate.
- Experience and knowledge of high utilizers, adults experiencing homelessness, SUD, incarceration, and mental health issues.
- Knowledge and experience with entry-level Microsoft Office and common personal computer functions such as email, printing/scanning documents, and web browser.
- Equivalent combination of training and experience to demonstrate the ability to perform the duties and responsibilities of the position.
- Licensed driver in the state of California.
- Ability to work in the community and a public or health care setting.
- Must pass a background check and drug test.
