BASIC PURPOSE: Provides clerical support to the Health Information Management department in the areas of medical record deficiency analysis, data abstracting, electronic data submission, release of information and maintenance of master patient index.
POSITION: SPECIFIC ACCOUNTABILITIES
Analysis/Reanalysis:
Selects appropriate assignments for analysis from work queue.
Reviews each record for missing documents, missing signatures or missing text according to procedure.
Assigns deficiencies to physicians according to procedure.
Maintains analysis turnaround within required time frame.
Assists with physician notification letters according to specified policy.
Answers any questions the physicians may have about their deficiencies and how to access assignments/complete deficiencies on HPF or directs to appropriate person for clarification.
Monitors reanalysis queue (physician decline queue) daily and reassigns deficiencies when appropriate.
Verifies physician dictation on the dictation system if physician indicates delinquent records have been dictated and dictated report is not in HPF.
Assists with the preparation of the physician suspension list according to procedure.
Performs reanalysis process accurately.
Uses initiative and creativity to encourage physician compliance with chart completion policies.
Assists others with responsibilities and adjusts work schedule to meet department needs.
Birth Certificates:
Performs duties to complete and submit birth certificates and fetal death certificates according to standards set by the Ohio Department of Health, with a 99% accuracy rate.
Interviews mothers, if applicable, obtains signatures of obstetricians and parents, as required.
Submits completed certificates to the State within 10 days.
Trauma Registry:
Performs duties to complete electronic trauma registry program according to the standards set by the Ohio Department of Safety.
Monitors inpatient and emergency room records for diagnosis codes related to trauma, only on those cases where the patient was admitted from the emergency room or transferred to another acute care facility due to trauma; submits trauma data to the State on a quarterly basis.

Master Patient Index:
Release of Information:
Processes release of information requests from the official medical record following hospital policies and procedures, State and Federal regulations.
Reviews each request to determine need for authorization and adequacy of authorization.
Maintains expected turn around times of release of information requests.
Responds to subpoenas, appearing in court as needed
Utilizes HPF Release Of Information [ROI] module for both electronic and hard copy medical record requests
Responsible for retrieving information off microfiche/film.
Generates reports out of HPF as needed to track ROI volumes, turn around times, etc.
Other:
Performs other duties as assigned.
EDUCATION, SPECIALIZED AND/OR TECHNICAL KNOWLEDGE REQUIREMENTS:
High school graduate or equivalent.
