We are looking for a full time or part time RN or LSW Case Manager for our facility in Indianapolis.
Kindred Hospital Indianapolis North is a 45-bed long-term acute care hospital we offer the same critical care patients receive in a traditional hospital or intensive care unit, but for an extended recovery period. Our team plays a vital role in the recovery process for chronic, critically ill and medically complex patients. We are located within American National University community at: 8060 Knue Road, Indianapolis, IN 46250.
Our Registered Nurse/Social Worker Case Manager provides oversight and intervention through assessing, planning, coordinating, monitoring and evaluation of services required to respond to individual patient needs. Responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a discharge plan in collaboration with the multidisciplinary team.
- Responsible for utilization review, discharge planning and care coordination with the patient, patient’s family, physician and health care team.
- Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
- Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family.
- Coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition.
- Documents professional recommendations, discharge plan, care coordination interventions, and case management activities to effectively communicate to all members of the health care team.
Qualifications
- Associate Degree in Nursing or Bachelor’s Degree in Nursing, BSN-Preferred
- Currently licensed as a Registered Nurse in the state of practice
- 2 years experience in case management OR 3+ years experience in clinical nursing
- Certification in Case Management, Preferred
