JOB SUMMARY
Identify the need for medically related social services and pursue the provision of these services. This position is responsible for identifying the psychological, social and emotional needs of residents and devising and implementing services/interventions to meet those needs including collaboration with outside community resources.
KEY RESULT AREAS/ESSENTIAL FUNCTIONS OF THE JOB
1. Provide support and advocate for resident/patient and/or family.
2. Maintain knowledge of community resources.
3. Identify and seek ways to support resident’s individual needs and preferences, customary routines, concerns and choices through assessments and care planning. Ensure interpreters are engaged when indicated for those who do not understand English.
4. Find options that best meet the physical and emotional needs of each resident/patient.
5. Assist residents/patients with financial and legal matters i.e.: POA, applying for Medical Assistance, referrals to lawyers, referrals to funeral homes for preplanning arrangements, guardian/conservator.
6. Identify, provide services, and conduct interventions for grief, bereavement and end of life issues.
7. Inform residents/patients, families and responsible party of their rights.
8. Participate in discharge planning, development and implementation of social service care plan including the coordination of services for those with behavioral and/or psychological issues.
9. Complete assessments per guidelines.
10. Assist in vulnerable adult maltreatment investigation, reporting and associate education.
11. Demonstrate knowledge of federal and state regulations as well as community policies and procedures as they apply to the position and participate in on-going education.
12. Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company.
13. Performs other duties, tasks and/or projects as assigned.
QUALIFICATIONS REQUIRED
- Bachelor’s Degree from a school accredited in the Council of Social Work education
- Licensed by the Board of Social Work in the State of practice, which must be in good standing
QUALIFICATIONS PREFERRED
- Hospital or nursing home experience
- Experience with geriatric population
