- Variety of assignments, procedures, and treatments – we serve patients in-home and also work with facilities to supplement their staff.
- One-to-one patient care with a customized Plan of Care for the patient
- Support and Oversight of patient care from our Director of Nursing
RESPONSIBILITIES
- Implementing the patient's Personal Plan of Care
- Providing companionship and assisting with personal hygiene tasks
- Preparing meals and ensuring proper nutrition for patients
- Administering medications as per prescriptions and reminders
- Assisting with transfers and ensuring safety during ambulation
- Transporting patients to medical appointments as needed
- Completing errands such as shopping and other tasks on behalf of patients
JOB SKILLS AND REQUIREMENTS
- Demonstrated general knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology.
- General knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
- Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
- Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.
- Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
- Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
- Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
- Ensure the privacy of each patient’s protected health information (PHI).
- Build collaborate relationships with peers and other healthcare providers to achieve departmental and corporate objectives.
QUALIFICATIONS & REQUIREMENTS: Successful candidates must meet the following requirements:
- Pre-placement TB Test
- Pre-placement Physical Examination
- Current CPR preferred
- Own vehicle
- Current California Driver’s License
- Current Auto Insurance
- Verification of US Citizenship
- Must pass the criminal background check
- Must pass the drug test
- 1 year of experience as a HH LVN
