Responsibilities:
- Provide age and functionally appropriate program activities that include but are not limited to assistance, training, and general support.
- Assist in the development and implementation of goal plans as directed.
- Correctly and accurately record an individual’s activity and progress on service notes, goal and behavior plans, community outing sheets, and other forms as required.
- Assist individuals with purchases and maintenance of funds.
- Assist individuals with everyday activities such as food preparation following nutritional requirements and proper food storage guidelines, kitchen duties, teaching of appropriate skills, and monitoring eating.
- Follow daily personal care plans for the individuals such as bathing, toileting, grooming, and dressing.
- Assist individuals in room care and general house cleaning.
- Complete assigned housekeeping tasks and maintain general upkeep of the home and document thoroughly in the cleaning checklist.
- Supervise general group activities for the individuals
- Report medical observations and concerns to the house manager.
- Document all medical occurrences.
- Handle emergencies based on organizational protocols.
- Once fully trained in medication administration, you will be responsible for properly administering medication to individuals as prescribed by their providers.
- Communicate with the house manager, assistant manager, lead staff, and other staff through the staff book.
- Transport individuals to and from activities and other programs, using company vehicles and personal vehicles as necessary.
- Assist with maintenance of agency vehicles.
- Attend mandatory staff meetings and in-service training as scheduled.
- Participate and complete at least 24 hours of training annually as required by regulations.
Minimum requirements:
- Must be at least 18 years of age or older
- Valid Driver’s License
- High School Diploma or General Education Development (GED) certificate
Preferred, but not required:
- Prior experience working with individuals with intellectual disabilities
- Home Health Aide/Home Care
- Direct Support Professional experience in other settings
- Personal Care Assistance (PCA)
