The Position:
We are looking for an experienced and passionate Clinical Liaison for our Florida Market. The Clinical Liaison is responsible for marketing and business development within the Pediatric Health Care Community in which we serve. Building and maintaining key decision maker relationships, managing and processing patient referrals, ensuring all necessary paperwork is completed, and overseeing the patient's admission into our centers.
This role requires a proactive, organized, and empathetic individual who can effectively communicate with external healthcare agencies, patient family, healthcare providers, and internal staff. The ideal candidate will hold at least an LPN certification and possess a strong understanding of clinical processes.
Responsibilities include:
- Coordinates and assists with admissions to ensure and coordinate growing center census in our three Florida Panhandle locations (Crestview and Pensacola) as well as our center in Naples.
- ·Interprets HCK policies and governmental regulations, related to admissions, care plans and clinical staff.
- Manages HCK admissions process for all referrals.
- ·Daily communication with assigned communities to monitor availability, planned admissions, discharges and any barriers or requirements for admissions.
- ·Communicate referral status to local and corporate leadership and disseminate necessary documents for admission.
- Coordinate with healthcare providers and internal staff to ensure smooth transitions for patients.
- Track and follow up on referral statuses to ensure timely admissions.
- Travel twice per month to Naples, FL to actively manage marketing, key relationships, processes and patient transitions.
- Maintain accurate and detailed records of all referral sources, referrals and admissions.
- Provide excellent customer service and support to local leadership, patients and their families throughout the referral and admission process.
Requirements:
- Graduate of an accredited school of nursing.
- Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Florida is required.
- Five (5) years of proven ability and experience with marketing and coordinating admissions in a healthcare facility.
- Experience with Medicaid eligibility.
- Experience in post-acute healthcare setting, clinical assessments, patient care plans, and financial requirements preferred.
- Strong organizational and time management skills.
- Excellent communication and interpersonal skills.
- Ability to travel 50% is required.
- Proficiency in using healthcare management software and other relevant tools.
- Knowledge of relevant healthcare regulations and policies.
Physical Requirements:
- Ability to travel 50% of the time is required.
- Ability to work in a clinical environment.
Work Environment:
- This position is based in the Florida Panhandle with regular travel to Naples.
- The role involves working closely with healthcare providers (external), patients, and internal staff.
