America's Home Health Services is searching for an experienced Care Coordinator to assist the branch and team with growing the home care business in Dauphin, Cumberland and surrounding counties as well as establish strong, professional relationships with our wonderful team of caregivers.

Responsibilities:
- Responsible for promptly answering incoming calls and providing information and problem resolution in a friendly, professional, and knowledgeable manner
- Demonstrates commitment to excellent internal and external customer service
- Process payroll weekly and ensure minimum timesheet usage
- Be knowledgeable about all required documentation clients and caregivers require per the Department of Health, OLTL and additional regulatory organizations
- Oversee general branch functions and maintain a safe and clean office
- Order office supplies and maintain inventory on office and PPE products
- Be highly active in all software solutions at all times
- Assist team with reaching weekly goals
- Coordinates the admission and readmission of clients with Home Care Consultant
- Data entry of new client and caregiver information into software solution
- Provide effective communication with clients, caregivers, colleagues, other professionals and referral sources
- Calls clients/caregivers and families to verify demographic information, primary care physician, and to schedule admission date
- Creatively responds to patient and agency needs in a prompt and professional manner
- Compiles admissions packets
- Cross trains in other office staff positions to fill in, as necessary
- Assures timely completion of individual staff schedules
- Completes in-services within established deadlines
Requirements:
- Must be computer savvy and work quickly between multiple portals
- Demonstrate capability maintaining strict confidentiality
- Well organized, accurate, and attentive to detail
- Proven record of professional longevity
- Drivers license
