Beacon seeks to fill 2 full-time (40 hours/week) and 1 part-time (20-25 hours/week) Case Manager positions within our agency.
The first position is full-time and works with individuals and families in our Permanent Supportive Housing program. The Case Manager's primary responsibility will be to work with residents in the Crawford Homes Program to provide crisis resolution and flexible, intensive services related to client needs and goals in service of stabilizing and retaining housing. Must maintain accurate documentation and recordkeeping in an online database in keeping with grant stipulations. The position will involve transportation to appointments.
The second position is part-time with Beacon’s Rapid Re-housing Program. The Housing Case Manager is responsible for supporting tenants with their efforts to obtain and remain housed through Beacon's rapid re-housing program. The program extends to Shalom clients and people experiencing homelessness throughout a six-county region. Reliable transportation is required. The Case Manager is responsible for documenting all service activities as defined by the Center’s procedures and policies and any grants' stipulations.
The third position is a full-time Crisis Case Manager at our Day Center to support our work with adults experiencing extreme poverty, especially people experiencing homelessness. The Case Manager will do responsive case management including conducting assessments, identifying and accessing resources, and developing case plan goals with guests; crisis intervention including the ability to remain calm in emotionally difficult situations and to de-escalate behaviors; and management of compliance paperwork. The work will be based at the Shalom Center.

Preferred qualifications: Bachelor’s degree in social services. 2 years working directly with individuals who have a serious mental illness and/or co-occurring substance use disorder. Case management experience.
Required: One year of experience working directly with vulnerable populations such as individuals experiencing homelessness or adults with disabilities. Excellent interpersonal and communication skills. Intermediate proficiency in computer skills. Must have a driver’s license and access to a vehicle.
