RESPONSIBILITIES:
The prime responsibility of the LVN is to provide direct care and supervision for each consumer; and supervision over Direct Support Professionals (Certified Nursing Assistants); and supervise other allied medical support staff.
This job classification has been identified as having a risk for acquiring an occupational exposure to blood or other potentially infectious materials. Protective equipment such as gloves, gowns, masks/face shields should be worn when exposure to blood or other potentially infectious materials is anticipated. Clinical judgment should be exercised when deciding what type of protective equipment to wear.
Responsibilities include, but are not limited to the following:
1. This position is a mandated reporter, and is expected to observe all consumers rights, report unusual occurrences and suspected dependent adult abuse, as specified in Title 22, California Code of Regulations.
2. An LVN staff is expected to awake while on duty.
3. Practices nursing care within the scope of the CA Board of Vocational Nursing and Psychiatric Technicians.
4. Maintains confidentiality and compliance with HIPAA.
5. Performs and/or directs nursing procedures, such as administering medications and treatments including oral medication, hypodermic injections, urinary catheterization, enemas; taking and recording temperature, pulse, blood pressure, respiration and other health care procedures authorized within the scope of practice for the Vocational Nurse license and according to physicians’ orders and instructions.
6. Maintains the health of persons served by conducting physical assessments, monitoring health and dietary needs.
7. Provides supports to ensure all basic needs, i.e., grooming, hygiene, provision of hydration and nutrition, medication assistance are met in a timely, skilled and thorough manner.
8. Seeks an RN to intervene where there is need for starting, monitoring or administrating medication into an intravenous (IV) line.
9. Initiates CPR and first aid as needed.
10. Assures the delivery of health-related services and provides nursing services to maintain health, maximize independence and support.
11. Oversees care and conducts training and on-going development by providing day to day supervision of direct care staff. Monitors the activities and documentation of other staff for accuracy and completeness and follow up as necessary.
12. Participates as a member of the Interdisciplinary Team in all aspects of the development and implementation of the Individual Program Plan. Evaluate and revise healthcare status and health care plans to address all priority healthcare needs. Assists staff in occupational, recreational, vocational and educational programs for consumers.
13. Observes and documents consumers’ physical condition and behavior and reports significant changes to appropriate staff.
14. Communicates regularly, and as necessary, with the family member(s), guardian(s) and significant others of persons served.
15. Works with consumers in activities that provide opportunities for learning and practice of skills tailored to the specific needs of the individuals.
16. Provides one-to-one supervision/direct observation of consumers on special occasions and precautions, i.e. an outing activity, elopement risks, suicide risks and medical condition and risk of serious self-injurious behavior to client.
17. Coordinate all healthcare-related appointments and other activities; provides transportation and accompanies persons to and from appointments and activities.
18. Evaluate each consumer’s status to ensure that needs are being met in a continuous manner.
19. Maintains a safe, sanitary, and therapeutic environment which promotes respect and dignity and protects the privacy, rights, confidentiality and physical/emotional well-being of the consumers.
20. Responds to emergencies that involve the use of medical/behavioral intervention techniques.
21. Participates with consumers on appointments, activities and outing in the community as scheduled.
22. Works extended hours and/or varying shifts. Works at other various homes operated by LSA, within Santa Clara County, as needed to meet staffing requirements.
23. Prepares and meets health care objectives.
24. Attends and participates in training as required.
25. Other duties as requested.
Requirements as Outlined by Title 22:
1. Holds a current Vocational Nurse license issued by the CA Board of Vocational Nursing and Psychiatric Technicians and is CPR and first aid certified.
2. Has previous experience working with developmental disabilities; if not, must take 12 continuing education training hours within the first 6 months.
3. Department of Justice (DOJ)/Federal Bureau of Investigation (FBI) fingerprint and criminal record clearance.
4. Health Screen and TB clearance not more than 1 year prior or 7 days after employment.
5. Holds a high School diploma or GED certificate.
6. Is a minimum of 18 years old
7. Displays proficiency in written and oral English.
8. U.S. citizen or alien authorized to work in the U.S. with legal documentation.
Requirements as Outlined by Title 17:
1. Attends Direct Services Professional (DSP) Part 1 within the 12 months of employment, and Part 2 within 24 months.
Requirements as Outlined by LSA:
1. Holds a valid California driver’s license and auto insurance with a good driving record.
2. Demonstrates caring, patience, good customer service skills, and ability to respond to consumers and co-workers in an empathetic manner.
3. Risk/Safety Conditions: Will have on-going interaction with residents. Potential exposure of communicable diseases, blood-borne pathogens, medicinal preparations, and other conditions common to this work environment. May require some exposure to communicable diseases or bodily fluids. Essential physical activities: To include walking, sitting, hearing, and seeing up close, talking, standing, finger movement, repetitive motions. Is required to lift a maximum of 25 lbs. and will ask for assistance as needed lifting over 25 lbs. May be required to repetitively lift unassisted transfers of adults with adaptive equipment. Pushing and /pulling one wheelchair/gurney with up to 250 pound individual.
4. Maintains sufficient strength, agility and endurance to perform the duties contained in this job description.

(a) Each ARFPSHN shall do all of the following:
(1) Meet the minimum requirements for a Residential Facility Service Level 4-i pursuant to Sections 56004 and 56013 of Title 17 of the California Code of Regulations, and ensure that all of the following conditions are met:
(A) That a licensed registered nurse, licensed vocational nurse, or licensed psychiatric technician, is awake and on duty 24-hours per day, seven days per week.
(B) That a licensed registered nurse is awake and on duty at least eight hours per person, per week.
