We are seeking a dedicated and compassionate Clinical Nurse Coordinator to join our team. The Clinical Nurse Coordinator plays a vital role in ensuring the smooth operation of our clinic by managing administrative tasks, coordinating patient care, and facilitating communication between healthcare professionals and patients. This position requires excellent organizational skills, strong attention to detail, and the ability to empathize with patients experiencing chronic pain while adhering to clinic policies and procedures. This role will report to the Remote Clinic Director and On-Site Clinic Business Director.
Responsibilities:
1. Patient Care Coordination:
- Coordinate referrals to specialists and ancillary services as needed.
- Act as a liaison between patients and healthcare providers, addressing patient inquiries and concerns promptly and professionally.
- Ensure accurate and complete documentation of patient information in electronic medical records, including insurance, contracts, consents, and medical history forms
- Administer screenings and tests (ie cognitive function)
- Administer prescribed injectable medication as needed
- Support LPN with vitals and blood draws as necessary
- Coordinate with prior authorization contractor for expedient processing
- Communicate with pharmacies via phone and fax
2. Administrative Support:
- Ensure clinic is staffed to appropriate levels
- Facilitate clinic daily meetings
- Maintain and update patient records, ensuring compliance with HIPAA regulations
- Act as liaison between clinic and lab
- Coordinate with clinic staff to ensure seamless patient flow and continuity of care
- Order medical and clinic supplies as needed
- Train staff and manage daily staff activities
- Complete On-boarding and training of new clinic staff
- Review staff performance, implement improvement plans, and ensure clinic is staffed appropriately
3. Patient Education and Support:
- Provide patients with information about their treatment plans, medications, and self-care techniques
- Research holistic wellness methods to add to clinic wellness initiative
- Offer guidance and resources to help patients manage their pain and improve their quality of life, including providing counsel regarding patient incentive wellness programming
- Conduct follow-up calls or emails to assess patient satisfaction and address any issues or concerns
- Documentation of Incidents
4. Quality Improvement:
- Participate in quality improvement initiatives aimed at enhancing patient care and clinic efficiency
- Identify areas for process improvement and suggest solutions to optimize workflow and patient experience
- Collaborate with healthcare providers and staff to implement best practices and protocols
- Review and ensure adherence to compliance policies and procedures
Qualifications:
- BSN or ASN.
- Proven experience in a healthcare setting, preferably in pain management or a related specialty.
- Strong understanding of medical terminology, procedures, and electronic medical records systems.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare professionals.
- Proficiency in computer applications, including MS Office Suite and electronic health record software.
- Ability to prioritize tasks, manage multiple responsibilities simultaneously, and work effectively in a fast-paced environment.
- Empathetic and compassionate attitude towards patients dealing with chronic pain and other health challenges.
