Community Horizons is passionate about supporting disabled and elderly individuals to live the life they choose. All the individuals that we support are enrolled in a self-directed program, funded through Medicaid. This program allows the recipient to be in charge of their own life and choose how they receive the care needed to live an independent life on their terms.
The hours for this position are between 8a-5p Monday-Friday. You have the privilege to work from home with company-provided technology (computer, scanner/printer, cellphone, etc.) Community Horizons has the ability to provide a flexible work schedule for those who may need a part-time work week. Who we are looking for:
- We are seeking someone who shares our passion for advocacy.
- Has a love for those with different abilities.
- Has exposure professionally to the recruitment process of caregivers. Minimum of 1-year experience is required.
- Has worked alongside or managed caregivers in long-term care facilities or home health agencies.
- Has worked with the elderly or disabled population for a minimum of 5 years.

Responsibilities of this this position:
- Attend in-person meetings with clients in their homes.
- Collaborate with clients and their teams to put together an effective recruitment strategy.
- Recruit caregivers through job boards, networking, and social media.
- Coordinate and schedule interviews.
- Assist with the onboarding process.
- Chart all support provided daily.
About 30%-40% travel required within Outagamie and Winnebago Counties.
