RN Case Manager Responsibilities:
Responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician’s plan of treatment and orders.
- Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process. Maintain a caseload and productivity per agency’s protocol.
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Maintain regular communication with Director of Patient Services/designee to review caseload.
- Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.
RN Case Manager Qualifications:
- Possess a current RN license in the State of California.
- Minimum one year of experience as an RN.
- Willing to train caring and compassionate individuals
- Prefer Hospice, Home Health, Skilled Nursing, or Acute Care experience.
- Ability to work well within a team environment.
- Strong verbal and written communication skills.
- Must possess and maintain a current CPR certification
- Time management skills to manage visits, documentation, and meetings.
- Possess a valid California driver’s license, valid auto insurance, and an automobile for field use
RN Case Manager Physical Demands:
- Requires manual dexterity sufficient to provide hands-on care to the patient under variable conditions such as standing, bending, stooping, pushing, pulling, stretching, walking, and repetitive hand motion associated with keyboarding
- Ability to turn, reposition, and assist patients to transfer and ambulate
- Must be able to lift a minimum of 50 pounds
- Driving or riding in a motor vehicle
- Requires normal ranges of hearing and vision with/without corrective aid
Service areas: Walnut Creek, Concord, Pleasant Hill, Martinez, Brentwood, Pleasanton, Livermore, Hayward, San Lorenzo, Sunol, Union City, Fremont.

Pay: $56.00 - $72.00 per hour
Expected hours: 40 per week
