Champion Healthcare of North Texas a leading provider of hospice care throughout Dallas-Fort Worth, TX. We are seeking qualified Hospice Aides to join our team in Dallas-Fort Worth, TX.
As a Hospice Aide you will be responsible for the following:
- Provide direct patient care under the supervision of the Clinical Director or the Director of Patient Care Services (PCS) to meet the needs of the client, including personal care, health monitoring, and terminally ill patients.
- Maintain accurate records of patient care activities and related incidents; prepare weekly weekly summary report; and conduct quality audits as needed.
- Participate in patient and family meetings to plan family care activities and participate in the evaluation process.
- Assist patients with self-administration of medications and treatments.
- Assist with other duties as assigned.
Required Skills:
- Current CPR certification (preferred)
- High School Diploma or GED
- Valid driver’s license with an acceptable driving record
- Ability to pass a background check and a drug screen, as well as proof of full Covid-19 vaccination, or willingness to be vaccinated
- Ability to work independently, and as a team member, with minimal guidance from a supervisor, manager, or other employees.
- Excellent communication skills (written and oral)
- Ability to communicate effectively with diverse populations and individuals from diverse backgrounds.
- Demonstrated ability to work independently and take direction when necessary.
- Demonstrated ability to work well under pressure in a fast-paced environment working with a deadline.
Responsibilities:
Under the supervision of the PCS Office Director, the Hospice Aide will be responsible for providing quality patient care under the direct supervision of the PCS Office Director. The Hospice Aide will also be responsible for maintaining an accurate patient chart and participating in all quality assurance programs as assigned by Champion Healthcare of North Texas Hospital.
Pay: $17.00 - $23.00 per hour
