Join our nonprofit client in Los Angeles as a Case Manager and play a pivotal role in promoting children's emotional well-being. You'll lead groups for children, high-risk teens, and adult parents, focusing on healthy relationships and positive parenting. This role champions a strengths-based approach, supports community-based services, and aligns with our mission and values.
Key Responsibilities:
- Conduct needs assessments with therapists to connect clients with resources like health insurance and housing.
- Provide individual and group rehabilitation services under various funding contracts, meeting productivity standards.
- Maintain timely documentation via our electronic health record system (EXYM) and ensure accurate progress notes and insurance details.
- Guide parents in managing child behaviors and familial stressors, identifying internal resources for support.
- Develop and deliver client rehabilitation services to enhance daily functioning at home, school, and in the community.
- Coordinate consultations, communications, and referrals across agencies.
- Attend community meetings and represent Wellnest at resource fairs.
- Follow up with families who discontinue treatment, providing ongoing support.
- Deliver services in homes, schools, and community settings.
- Participate in assigned trainings and meetings for professional development.
Required Qualifications:
- 1-2 years of case management experience with at-risk populations.
- Bachelor’s degree in social work, psychology, or related field.
- Valid CA driver’s license and insurable driving record.
- Completion of TB test and DOJ fingerprint clearance.
