ESSENTIAL POSITION DUTIES:
Completes and validates Medicare, Managed Medicare Assessments in accordance with RAI manual guidelines.
Schedules and maintains ARD Calendar for assessments
Coordinates ARD selection for Medicaid CMI
Administers appropriate denial notices, SNF ABN, NOMNC, Detailed notices
Coordinates care plan reviews and completion
Assigns active and historical diagnosis coding of residents
Reviews and updates CMI assessment reports for validation.
Schedules and facilitates Interdisciplinary Team Care Plan meetings
Coordinates and submits Managed Care Insurance company updates
Tracks and manages ASCEND approval status and submits required changes
Provides ADL or MDS driven education as needed with Staff Development
Reviews Quality Indicator, SNF VBP, and other Five Star quality reports
Participates in meetings or collaborative QAPI initiatives as assigned
PCC Experience required
Professional Experience/Educational Requirements
- Demonstrated experience in direct patient care in a medical, surgical, long-term care setting with responsibility for multiple patients.
- RN Degree
- On-call required
- Demonstrated proficiency in RAI/PPS process.
Certification/licensure
- Current CPR certification
- Current RN license in State of Connecticut
- RACC certification preferred
